Our office is proud to provide evaluations and treatment management for our patients 6 year old and up with Attention Deficit Hyperactivity Disorder (ADHD).
If you are interested in having your child evaluated, we require parent and teacher questionnaire forms (Vanderbilt forms) to be filled out before scheduling your appointment.

As part of our ADHD management plan once your child is diagnosed, we will need your child to have yearly physicals and we will see your child every three months for routine medication checks. You will also need to provide completed Vanderbilt forms at least twice per year. Due to the nature of some of the medications used to treat ADHD, if medications are part of your child’s treatment plan, we require a visit with the cardiologist for medication clearance.
Your provider may also ask you to have a more formal evaluation done with a psychologist if there are any risk factors associated with ADHD such as anxiety, depression, or oppositional defiant disorder.
